Zvýšenie intrakraniálneho tlaku je jedným z najzhubnejších patofyziologických mechanizmov po úraze hlavy. Jeho terapeutické ovplyvnenie možno dosiahnuť rozličnými spôsobmi. Z chirurgických metód je popri evakuácii poúrazových intrakraniálnych hematómov veľmi účinná aj vnútorná, vonkajšia alebo kombinovaná dekompresia pri expanzívne sa správajúcej kontúzii mozgu. O výsledku chirurgického liečenia rozhoduje hlavne stav chorého tesne pred operáciou.
