Pozdní diagnostika páteřního postižení u myelomu

Autoři retrospektivně analyzují skupinu 19 pacientů operovaných pro mnohočetný myelom a plazmocytom páteře a rozebírají obtížnost správné a včasné diagnostiky. Prvním projevem onemocnění byly bolesti zad, které se vyskytovaly u 12 pacientů, tj. v 63 % pacientů. Předcházely operačnímu řešení průměrně 4 měsíce před stanovením diagnózy. Ve sledovaném souboru bylo 17 operovaných pacientů, tj. 89 %, přijato k operaci pro maligní onemocnění páteře bez znalosti základního hematologického onemocnění. Předoperačně se vyskytovaly klinické projevy míšní komprese u 12 pacientů, tj. v 63 % souboru, které se začaly rozvíjet průměrně 19 dní před operací. Operačním výkonem jsme dosáhli zlepšení neurologického stavu u 7 pacientů, tj. v 58 %, s předoperačním neurologickým deficitem.
