HIV encefalopatie

HIV (human immunodeficiency virus) encefalopatie a progresivní multifokální leukoencefalopatie (PML) patří mezi progresivní onemocnění centrálního nervového systému u pacientů s AIDS. U HIV encefalopatie je podkladem onemocnění infekce centrální nervové soustavy virem HIV a u PML jako hlavní příčina reaktivace latentní papovavirové infekce (JC virus). Odhaduje se, že HIV encefalopatií je postiženo v průběhu AIDS cca 5–19 % pacientů a PML cca 2–5 % pacientů. Tato onemocnění se souhrnně vyznačují progredujícím neurologickým deficitem (u HIV encefalopatie spíše symetricky vyjádřeným, u PML lze najít fokální neurologický asymetrický deficit), na magnetické rezonanci jsou nacházena hyperintenzní ložiska v T2W (u PML charakteristicky multifokální, u HIV encefalopatie charakteristicky difuzně splývavá), laboratorně je nacházena leukopenie, v likvoru obraz serózního zánětu. Autoři prezentují vedle přehledu z literatury kazuistiku 32leté ženy, u které vedl souhrn klinického obrazu, laboratorních a zobrazovacích metod k diagnóze AIDS.
