Akutní diseminovaná encefalomyelitida a její možná záměna s AIDP

U 21leté pacientky se 4 dny po respirační infekci rozvinula paraparéza dolních končetin s poruchou čití na trupu a dolních končetinách. Nemocná byla přeložena na naše neurologické oddělení k plazmaferéze pro akutní polyradikuloneuritidu. Po překladu jsme nejprve provedli vlastní EMG vyšetření, které nesvědčilo pro neuropatii. Následně jsme indikovali MR míchy i mozku s nálezem svědčícím pro ADEM. Diskuze je zaměřena jednak na diagnostická úskalí ADEM, diferenciální diagnostiku vůči akutní polyradikuloneuritidě i na terapeutické problémy.
